Services and Merchandise Offered
The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If you are required by law to buy items which you do not specifically request, we will explain the details fully on your statement of funeral goods and services.

| Minimum Services of the Funeral
Director and Staff |

This fee for our basic services and overhead
will be added to the total cost of the funeral arrangements you select.
(This fee is already included in our charges for direct cremations,
immediate burial, and forwarding or receiving remains.) This fee includes
the following:
- Personnel available 24 hours a day, 365 days a year to respond
to initial call
- Arrangement conference
- Coordinating service plans with cemetery, crematory and/or other
parties involved in the final disposition of the deceased
- Clerical assistance in the completion of various forms associated
with a funeral
- Securing and recording death certificate and disposition permit
- Also covers overhead, such as facility maintenance, equipment
and inventory costs, insurance and administration expenses, and
government compliance

| Preparation of the Body
|
|
|
Embalming: Except in certain
special cases, embalming is not required by law. Embalming may be
necessary, however, if you select certain funeral arrangements, such
as a funeral with a viewing. If you do not want embalming, you usually
have the right to choose an arrangement that does not require you
to pay for it, such as direct cremation or immediate burial. |

Refrigeration: Our
policy requires refrigeration for an unembalmed body after 8 hours.
This refrigeration fee is for a 3 day period or portion thereof. Each
additional day costs $50. |

Our services include coordinating the funeral arrangements, supervision
of funeral, display of floral arrangements, and staff assistance with
funeral ceremony. |

home, church or other location: Our services include
coordinating the memorial service arrangements, supervision of the
memorial service, display of floral arrangements, and staff assistance

of remains to cemetery, supervision of graveside services, display

you can use an alternative container. Alternative containers encase
the body and can be made of material like fiberboard or composition
materials (with or without outside covering). The containers we provide
are fiberboard. |
